各學(xué)院、全體在校學(xué)生：

根據(jù)中央、四川省等有關(guān)部門文件、會議精神和學(xué)校黨委、行政部署，為加強對寒假留校學(xué)生的疫情管控工作，最大限度阻斷疫情傳播擴散渠道，減少學(xué)生外出被疾病傳播的風(fēng)險，保障全體學(xué)生的身體健康和生命安全，學(xué)校決定，在疫情防控期間加強校園出入管理，實施學(xué)生外出審批制度，現(xiàn)將有關(guān)事宜通告如下：

一、學(xué)校要求在校學(xué)生非必要不外出校園，避免到空間封閉、通風(fēng)不暢和人流密集場所活動，避免與有癥狀的人員接觸。

二、確有特殊原因必須外出的，須到學(xué)院老師處填寫《疫情防控期間學(xué)生外出審批表》（附件1、2分別為中國學(xué)生版、國際學(xué)生版），經(jīng)簽字蓋章后會同個人有效證件（學(xué)生證或校園一卡通）進出校園。

三、外出期間自覺遵守國家法律法規(guī)和校紀(jì)校規(guī)，切實做好應(yīng)對疫情的個人防護，確保通訊暢通，自覺注意人身及財產(chǎn)、交通、飲食、消防等安全。

四、按時返校并向?qū)W院銷假，返校后及時洗手、洗鼻，身體如有不適及時就醫(yī)并向?qū)W院和校醫(yī)院報告。

Announcement on the Implementation of the Approval System for Students Going Off-Campus during the Prevention and Control of Pneumonia Epidemic of New Coronavirus Infection

All students staying on campus:

In accordance with the documents of the Central Government, Sichuan Province and other relevant departments, the spirit of the conference, and the university committee and administrative arrangements, in order to strengthen the epidemic prevention and control of students staying on campus in the winter vacation, block the spread of the epidemic to the greatest extent, reduce the risk of the spread of the disease during students going out, and protect the health and safety of all students. The university has decided to strengthen campus access control during the epidemic prevention and control period, and implement a student going out approval system. The relevant matters are now notified as follows:

1. The university requires students not to go out of the campus unless necessary, to avoid closed, poor ventilation and crowded places, and to avoid contact with symptomatic personnel.

2. If there is a special reason to go out, the student must go to the teacher of the school to fill out the Approval Form for Students Going Out during the Prevention and Control of Pneumonia Epidemic of New Coronavirus Infection (see attachment for details). After signing and stamping, you shall be allowed to enter and exit the campus with your valid personal certificate (student ID or campus card) .

3. Consciously abide by national laws and regulations and university’s discipline and regulations during going out, and do a good job of personal protection against epidemics, ensure smooth communication, and consciously pay attention to personal, property, transportation, food, fire and other safeties.

4. Return to the university in time and report to the school. Wash hands and nose in time after returning. Seek medical treatment and report to the school and university hospital if body feels unwell.
